A client is being discharged with a prescription for chlorpromazine (Thorazine). Before the client leaves for home, which finding should a nurse teach the client to report within 24 hours?

1. Change in libido, breast enlargement

2. Sore throat, fever

3. Abdominal pain, nausea

4. Dyspnea, nasal congestion

  1. # First look at what the question is asking you. The question is asking you to identify what sign the nurse should teach the client to report within 24 hours.
    # Look for clues and then start the process of elimination.
    # choice 1. doesn't sound like it is important or an emergency or something a person should report immediately.
    # choice 3 is also not that important
    # So, you are left with choices 2 and 4.
    # Look at choice 4, it has two parts. dyspnea is important but nasal congestion is not a major concern therefore you are left with choice 2 because it is the most important of all the four choices.
